The community hub aims to signpost patients and local residents to services and support for a variety of needs, including:
- Abuse.
- Access to transport.
- Accommodation/tenancy support.
- Bereavement.
- Digital inclusion.
- Family support.
- Food (foodbanks, cooked meals, classes, etc.).
- Employment support.
- Homelessness.
- Lifestyle problems.
- Loneliness/isolation.
- Mental and physical health support.
- Money support and advice.
- Parenting.
- Practical support.

Local Community Services
Well Aware is the free information and signposting service for everyone in:
- Bath & North East Somerset
- Bristol
- North Somerset
- Somerset and
- South Gloucestershire
The Care Forum and three local authorities and the BNSSG integrated care board have come together to develop a comprehensive database of health, wellbeing and community resources. Well Aware has also received development funding from Transforming Local Infrastructure in North Somerset and Healthwatch Somerset.
Building on the work of previous projects such as Room 102 or Spot On, the open portal service contains the details of 1000s of different services, as well as a wealth of other useful information.
Well Aware is:
- Free to use
- Free for organisations to be listed on
- User friendly, making it easy to find information you need for a friend, family member, client or patient – or for yourself
Well Aware also has dedicated online sections relating to:
- Mental Health and Employment
- Learning Difficulties
- Low Vision/Physical and Sensory Impairments
- Hearing loss
- Speakers of Languages Other Than English
