How to register with the Practice

We are using a new online service called https://gp-registration.nhs.uk/L81087  that makes it easy to register with this GP surgery.

Just fill in this quick online form by clicking link above to start the process. You do not need proof of address or immigration status, ID or an NHS number. You will need an NHS login to do this.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available from the surgery if you prefer this, you will be required to confirm your ID & address if using paper forms as usual.

Registering online as a non-English speaker

You can find guidance and advice on how to become a new patient and the NHS healthcare services you can access, if your first language is not English, or if you’re an asylum seeker on the Government website. 

Go to the NHS entitlements migrant health page on the Government website.

Registering as a temporary patient

If you are away from home, you can register as a temporary patient with Beechwood Medical Practice for up to three months. This will allow you to use our surgery and still remain a patient at your permanent GP surgery. After three months, you will have to re-register as a temporary patient or permanently register with us.

If you require services as a temporary patient, please call the practice reception.

Registering children

Children under 16 need to be registered by their parent or guardian. You can do this by completing the online GP surgery registration form on their behalf. You will need to provide your details as their parent or guardian.

What to do if you’re registering on behalf of someone else

If you are a carer of young children, elderly adults or adults who aren’t able to register themselves, you can set up an account on their behalf. This will allow you to book an appointment and access parts of their GP online services account.

Change of address

If you are moving house, please let us know as soon as possible. If you are moving out of the surgery’s catchment area, you may be asked to register with a surgery that’s closer to your new home address.
